Certificate of Insurance Requests
A certificate of insurance is a document that verifies the types of insurance coverage that is carried by the university. The certificate of insurance is issued by the University's insurance brokers. The Johns Hopkins University is often requested by contracts or outside parties to provide evidence of liability coverage.
If you need a certificate of insurance complete the Certificate of Insurance Request Form and return to the Office of Risk Management. If the other party is requesting to be added as an additional insured, please fax or email a copy of the contract to our office for review. Please do not use this form for new employer requests for proof of medical malpractice/professional liability insurance, please see the below instructions.
